Can you do Telephone Visits (tele-health)? ▿
Yes! You now have the option to make telephone appointments with your provider in the convenience of your own home for questions on your health or prescriptions in a tele-health visit.
Can I make same day/next day appointments? ▿
Yes, we do our best to accommodate same and next day appointments.

What types of health insurance do you accept? ▿
We accept Medi-Cal, Medicare, and most private insurances. In addition, UHC is a Veteran’s Choice Program Provider and we are proud to service the Veterans in our communities.
What if I don’t have health insurance? ▿
We are here to help! Our Enrollment Specialists can assist you with enrollment into Covered California Health Plans and Medical. In addition, we offer a Sliding Scale Fee Program for those without insurance. Please click here for more details.
What do I need to bring with me on my first visit? ▿
1. Official identification, which can include a social security card, a valid driver’s license, a birth certificate, or another form of official identification.
2. Your Medi-Cal, Medicare, or private insurance card.
3. Your medical records from previous medical care, unless they have been forwarded or electronically transferred from your previous provider prior to your appointment.
4. A current list of your medications.
What will happen during my first appointment? ▿
First, you will be asked to arrive 20 minutes early to complete the patient registration process.
Next, our wonderful medical staff will get your weight and blood pressure, and will ask questions regarding your medical history.
Lastly, you will be escorted to an exam room to see a provider.
