What is Enhanced Care Management (ECM)?

• ECM is a program designed to provide support to people with complex health needs. Our goal is to provide you with personalized support to manage your health and connect you to important services, so you can live a healthier life.

• ECM services are provided at no cost, and you join or stop ECM at any time.

Can I Enroll in ECM and Why Should I Self-Refer?

• If you feel like you need support with managing your health, ECM might be right for you.

• Whether you have a long term-illness, mental health challenges, or frequent hospital visits, ECM provides the support you need.

• By self-referring, you take charge of your health with personalized care from a dedicated team.

• We'll help you stay on track and connect you to services that fit your needs.

What Does ECM Offer?

• Personalized Care Plans: We'll work with you to create a plan that fits your health needs.

• Support from a Team: You'll have a care team, which may include Care Managers, Community Health Workers, doctors, nurses, and social workers, to help guide you.

• Connections to Services: We'll link you to doctors, mental health resources, and community services that can support your health.

• Ongoing Help: Your care team will check in with you regularly to make sure everything is going smoothly.

Who Qualifies for ECM?

• ECM is a Medi-Cal Managed Care Plan benefit. UHC is partnering with Health Net and Anthem BCBS to provide this service.

• If you are enrolled in one of the above health plans and meet at least one ECM Population of Focus, you may qualify for the program.

• Take the guess work out of it. You can self-refer and our team will screen you!
